Notifiable Health Conditions

Collection, investigation and distribution of data about illness and death helps prevent and control disease in Washington state.

Report Notifiable Conditions to SRHD

Please report the conditions listed below as noted in the "When to Report" column. When in doubt, please call and our disease investigators can assist:

  • Immediately Notifiable - call to report 24/7.
    Report by phone: Call 509.324.1442 and choose Option 2 to speak to staff (24/7)
  • 24 Hour Reportable - can report next morning, 7 days/week by phone or fax
  • Sexually Transmitted Infections - complete the STI Report Form (link below) and fax to the number on the form
  • All other conditions - use the CD Report Form for most diseases, unless a different form is linked below.
