Notifiable Conditions in Washington State
The following diseases and conditions are reportable in Washington state. Each disease or condition listing in the table below includes links to Washington State Department of Health Reporting Guidelines, the reporting time frame, and the form to use for each disease or condition.
Immediately Notifiable
For conditions that are considered immediately notifiable, report by calling the SRHD Communicable Disease Report Line at 509.324.1449, which is available 24/7. The primary form for most diseases is the Communicable Disease Report Form (CD) unless another form or reporting procedure is noted. If you have questions about whether to report a condition, please call 509.324.1442.
