Appointments are recommended, but not required. Walk-ins are welcome.
This COVID-19 vaccination clinic is open to the community for ages 12 and up. The following vaccines will be available:
- Pfizer-BioNTech vaccine – requires two (2) doses, for individuals age 12 and up
- Johnson & Johnson vaccine – requires one (1) dose, for individuals age 18 and up

Information for parents/guardians:
Individuals ages 12-17 years will need the consent of their parent/guardian before receiving the Pfizer vaccine. Parents and guardians can provide their consent during the registration process (see link to register below). Legal guardians do not need to be present in order for their child to receive the vaccine as long as there is documented consent.
If you need assistance with scheduling an appointment, please call 1.800.525.0127 and press # and let the operator know the date and location of the clinic. There will also be staff on site to assist with registration.
