Nurse-Family Partnership is an evidence-based home visiting program that matches eligible pregnant persons with a registered nurse who has received a bachelor’s degree and specialized training in the NFP model. Spokane County NFP has capacity to serve 175 families and is part of the NFPx initiative, which offers enrollment to pregnant parents with previous live births, as well as those who are past 28 weeks. However, earlier enrollment supports building trust and rapport between parents and their nurse home visitor.
NFP nurses are trained to support clients in all facets of life. They assist clients in learning how to advocate for themselves and their children and become their own change makers. Nurses tailor the amount of assistance and support they provide for each client, working to empower clients while supporting self-sufficiency. Nurses also support clients with care coordination between healthcare providers and community resources such as housing, education, and family support services. Walking alongside clients allows NFP nurses to evaluate barriers that prevent families from achieving their best health and wellness and work to breakdown those barriers and achieve the goals they’ve set for themselves.

NFP clients are eligible to participate in blood pressure self-monitoring at 20 weeks gestation through eight weeks postpartum. NFP of Spokane County is extending these efforts by purchasing blood pressure kits for clients with grant funds provided by the Washington State Department of Health’s (DOH) Washington Blue Band Initiative.
As a part of the initiative, NFP clients at risk of developing preeclampsia or elevated blood pressure are also given a blue wrist band to wear during pregnancy and after delivery to alert health care providers about clients’ risk for preeclampsia.
