Access Your Immunization Records

If you need to access your family's immunization records information, you have a few options. Immunization records are not maintained at the national level. Instead, states maintain this information in immunization registries. Washington state’s registry is the Washington State Immunization Information System (IIS), which is maintained by the Washington State Department of Health (DOH). Only healthcare providers can access the IIS, but individuals
have other options for accessing records for vaccines received in Washington state. Please note, records for vaccines received in other states are not available in Washington state systems.


OPTION 1: Use MyIR

The quickest way to access all records for vaccines received in Washington state is to sign up for Washington state’s free immunization record website MyIR Mobile. If you have trouble accessing vaccination records through MyIR, call 833-VAX-HELP for assistance.

OPTION 2: Call Your Provider or Pharmacy

Visit or call your usual location for receiving vaccinations. This might be your local pharmacy or health care provider.


OPTION 3: Request Records from Spokane Regional Health District

You can also request a complete immunization record from Spokane Regional Health District. To do this, you will need to fill out a Request for Access to Health Information Form. These are available in English, Marshallese, Russian, Spanish and Ukrainian.

*Parents cannot request on behalf of children over age 18. Individuals 18 years and older must submit their own form.
