SRHD is implementing a new system for LTCF to submit positive COVID-19 case reports for their residents and staff. This online reporting form will streamline both reporting for facilities, and processing reports for SRHD, by ensuring that information submitted is complete (as required by WAC 246-101-115) and in a consistent format. Please begin the transition to this new form immediately. We will no longer accept case reports in the line list formats previously being used as of Oct. 29, 2021.
Also attached is a chart explaining the recommended frequency of staff testing.
To assist you with the reporting transition, we are hosting a weekly online “office hours” - every Wednesday afternoon from 3 – 4 p.m. This will be an opportunity for you to join via a remote meeting link to ask questions and share concerns.
