Case Management

Spokane Regional Health District offers case management services to persons living with HIV/AIDS in the Spokane community. These services are provided free of charge in cooperation with the Washington Department of Health. HIV/AIDS case management assists individuals with locating, coordinating, advocating, and referring for medical, psychosocial, housing, and other services based on assessed needs. Case management plays a vital role in helping HIV positive individuals navigate and access the sometimes-challenging medical care and resources needed to stay healthy.

Summary Overview

Case management is a confidential, client centered process that links HIV positive individuals to services to ensure timely, coordinated access to medically appropriate levels of health and supportive services. Our professional team works with individuals to develop an action plan to assist in achieving and maintaining stability across a  multitude of life domains. Case management is for people living with HIV/AIDS who could benefit from support, advocacy, referrals, and linkage to health insurance and community resources, information and education, skill building or coordination of services around their health condition.

Eligibility for HIV/AIDS Case Management

  • Proof of HIV/AIDS diagnosis
  • Proof of Washington State Residency
  • Proof of household income
  • Proof of any health coverage status

The consumer advisory board (CAB) was created to provide input from our clientele regarding the design and delivery of our care services. SRHD HIV/AIDS case managers help those with HIV through the many challenges they may face in managing the disease. They help clients identify their strengths and areas of need, guiding them through, what is often, a maze of applications and referrals. Case managers assist clients with prescription drug coverage, housing, mental health care, and substance use issues. They help coordinate services, advocate for clients, and provide emotional support as needed.
