Case management is a confidential, client centered process that links HIV positive individuals to services to ensure timely, coordinated access to medically appropriate levels of health and supportive services. Our professional team works with individuals to develop an action plan to assist in achieving and maintaining stability across a multitude of life domains. Case management is for people living with HIV/AIDS who could benefit from support, advocacy, referrals, and linkage to health insurance and community resources, information and education, skill building or coordination of services around their health condition.

The consumer advisory board (CAB) was created to provide input from our clientele regarding the design and delivery of our care services. SRHD HIV/AIDS case managers help those with HIV through the many challenges they may face in managing the disease. They help clients identify their strengths and areas of need, guiding them through, what is often, a maze of applications and referrals. Case managers assist clients with prescription drug coverage, housing, mental health care, and substance use issues. They help coordinate services, advocate for clients, and provide emotional support as needed.
The Hepatitis AIDS Substance Abuse Program (HASAP) offers assessment, individual and group counseling, and referral to inpatient/outpatient chemical dependency treatment and other referral services.
Working with providers on the incidence, distribution, and possible control of diseases, illnesses and other factors relating to health.
